Because the cider-grove thing was both fantasy and reality. So I've got both bases covered. Like Kieth.
Start the ball rolling - do people still feel like the dream/fantasy sequences this season are OTT or give the show a distinct feel?

This is probably the issue that has the most love/hate relationship with fans. I for one enjoy all of them unconditionally, no matter what context, or degree of wackiness. I think ever since S4, almost everyone has their own dream sequences now and it just adds on the notion that this is truly a comedy show.
My favourite dream sequence has to be JD dressed up as Robin and Turk dressed as Batman.

You gotta love the classics:
J.D's fantasty of life as a surgeon - him and Turk enter the hospital blinged out in their pimp gear. Hee.
This is gonna total be vague but I love the fantasy when Todd rips out his heart for some reason and his last words are "I'm gay." I don't remember the context but I love it when the writers add little touches like that. Hee.

It's in, 403: 'My New Game', when Turk's lost the colonic cancer patient that JD's pulled his newly acquired Co-chief resident strings to hand to him.
Turk goes 'What more would the Todd have done?' or something along those lines and JD has the fantasy. Which Turk manages to intrpret from the look JD has on his face! Another awesome Scrubs touch.

I love love love JD's recent dream sequence about Turk doing exploratory surgery a la Raiders of the Lost Ark. It's the little touches: Turk muttering "The colon.... why'd it have to be the colon?", fluttering his fingertips as he's preparing to grab the Golden Tumor, then JD's "Watch out for colon darts"...

I like the dream sequences when they don't get too wacky. They don't have to be over-the-top to be hilarious. For instance, I love the visual depiction of the "Friend Zone" in my favorite episode, 1.03 "My Best Friend's Mistake."
The dream sequences as a whole haven't been annoying this season. But a few have gone on a little too long (Turk and Carla's pumpkin child) or crossed the fantasy/reality line a bit (J.D. fitting into Turk's backpack, J.D.'s "chest hands").

The backpack I'll give you, and maybe even the chesthands too, but the payoff of pumpkin baby was soooo good. It was all down to Faison & Reyes' reactions and the Lifetime parody :)

My favourite, hands down, was the one in the first season where JD lies in bed with Alex, his junkie girlfriend and she tells him that she's ready to start having sex with him and then he imagines that a marching band comes into the bedroom and plays some triumphal music, complete with a circus monkey enthusiastically banging the cymbals.
You can see this dream sequence coming from a mile away, doesn't make it any less awesome.

My favorite dream sequence was definitely in the first season (My Way or the Highway, maybe?), with the West Side Story parody of surgeons versus docs. Between the hallway dancing and the balcony singing, just plain hilarious.

Among my favourites - possibly my favourite one ever - was the sequence inspired by Dan's visit, where Dan tells JD he "just missed Turk and me harmonizing" and JD imagines the three of them as a sort of barbershop-quartet-style flatulence musical ensemble, complete with plaid vests and straw boaters. It's a great setup, but the added details, such as the audience reactions (clapping ecstatically while ducking the lower halves of their faces into their shirt collars; you can still see hints of sunny smiles on the faces in the audience) made it inspired. It's still the best example I can come up with for the writers' collective ability to put a high polish on a joke.
I also loved the extended "A Surgeon and A Doc" sequences based on West Side Story. I guess I'm a sucker for the musical touches.

I was watching "My Drama Queen" last night from Season 2, and I have to nominate the fantasy sequence where J.D. sees Elliott and Tasty Coma Wife talking to each other, and he has a fantasy of two girls doing the Re-Run dance. Hysterical. And then when J.D. approaches the girls and asks them what they're talking about, Elliott says "Not the Re-Run dance." Classic.

Two of my favorites are from the end of season two. The first is "My Dream Job" where J.D. has that dream of everything being chocolate, with "The Candy Man" playing in the background, and he was gnawing on everything. He ends up eating Turk's hand. That killed me.
The other one was in where J.D. fantasizes to everyone dancing to 99 Red Balloons, complete with...red balloons floating everywhere.

lulu I also love in that ep when JD has his bachelor party envisioned and all they guys do the rerun dance. The pose all the end where they all have their hands up kills me EVERY. TIME.
One of my favorites is the song at the end of "My Philosophy". When the actress starts in on "On a clear day..." I get teary no matter how many times I watch it.
I can seeeee, see for a very long way. Sniff.

Oh, my. You've really brought back memories now. I remember when I first watched that I was just so touched. Really an exception for sitcoms Scrubs is.
On a related note, I'd argue that one of the best fantasy sequences, probably the best is 'My Screw Up' itself. About half of the episode turns out to be in Cox's mind after all does it not? I'd be ashamed to admit it took me three rewatchings to figure out exactly what had happened if I weren't so glad that it did.

The whole song from 'My Philosophy' is great, but my favorite part from it, which cracks me up every time, is the Janitor. I think its probably because its couched in this serious-sad song with JD watching the whole production.

I love 'A surgeon and a doc' song on the balcony with Dr. Wen and Dr. Cox! Hee. I think that is my favorite dream sequence, just because it's Dr. Wen and Dr. Cox.
And now of course, the Grandfather Kennel in this week's episode.

I just saw the fantasy bit where Dr. Kelso and Elliot's dad square off, old-time gentleman pugilist-style. Awesome.
Also, as an actual dream, when Turk's fantasizing about Elliot and she wants to be spanked, the part where he licks his fingertips all in one swoop -- creepy hilarious.

Oh that was fantastic, but the best one was at the end when he confronts Dead!Husband!'s widow at the restaurant in front of his parents and then they all go into the 99LB mode! SO GOOD!

Pumpkin baby. (and JD's pissy "You're going to be a horrible father! as he storms off)
Or when the Janitor was talking about being trapped in the wall and thinking he was a mermaid. Hehe.
But probably the West Side Story one is my favorite.

I'm a sucker for the fantasy sequences that form the basis of an episode, so my favorite is in episode 3-16, "My Butterfly". It reminds me of when I was a kid and I'd read the Choose Your Own Adventure books, and I'd flip back and change what happened if I don't like the ending.
